Sure-Ski Season Pass Insurance You have two ways to report a claim with respect to your Sure-Ski Season Pass Insurance, they are: 1) print the online claims form, complete it and return it to the Claims Department of Industrial-Alliance Pacific Life Insurance Company at 2165 West Broadway, Vancouver, BC, V6K 4N5 within 45 days of continuous non-participation; or 2) call and report the claim directly to Industrial-Alliance Pacific Life Insurance Company at
Please note that you must advise Industrial-Alliance Pacific Life Insurance Company of
your claim for benefits within 45 days of you being unable to ski due to a covered condition.
